If we include as remakes later adaptations of the same play or book (as was True Grit), my guess is that there are 50 or more cases.

George Arliss/Disraeli (his second time playing the role on film)
    Greta Garbo/Anna Christie
    Frederic March/Jekyll & Hyde
    Katharine Hepburn/Alice Adams
    Miriam Hopkins/Becky Sharp
    Merle Oberon/The Dark Angel
    Basil Rathbone/Romeo & Juliet
    Norma Shearer/Romeo & Juliet
    Greta Garbo/Camille
    Barbara Stanwyck/Stella Dallas

    That’s just through 1939

Juliette Lewis was also nominated for that^

And Mark Walhberg was nominated for The Departed which is a remake of a foreign movie, the only remake to win BP, although I don’t know if his role was in the original
