Guest-acting awards were initiated in 1986. They first started off without separating the genders. In 1989 the current system we recognize took effect. Winners among the ladies guesting in comedy were:

 Beah Richards, Frank’s Place (CBS)
 Colleen Dewhurst, Murphy Brown (CBS)
Swoosie Kurtz, Carol & Company (NBC)
 Colleen Dewhurst, Murphy Brown (CBS)
suspended (guests in that year’s awards were combined with series lead and supporting players)
Tracey Ullman, Love & War (CBS)
Eileen Heckart, Love & War (CBS)
Cyndi Lauper, Mad About You (NBC)
Betty White, The John Larroquette Show (NBC)
Carol Burnett, Mad About You (NBC)
 Emma Thompson, Ellen (ABC)
 Tracey Ullman, Ally McBeal (Fox)
 Jean Smart, Frasier (NBC)
 Jean Smart, Frasier (NBC)
Cloris Leachman, Malcolm in the Middle (Fox)
 Christina Applegate, Friends (NBC)
 Laura Linney, Frasier (NBC)
 Kathryn Joosten, Desperate Housewives (ABC)
Cloris Leachman, Malcolm in the Middle (Fox)
 Elaine Stritch, 30 Rock (NBC)
 Kathryn Joosten, Desperate Housewives (ABC)
Tina Fey, Saturday Night Live (NBC)
Betty White, Saturday Night Live (NBC)
 Gwyneth Paltrow, Glee (Fox)

There used to be a sort of unofficial guest category called Outstanding Single Performance by an Actress in a Comedy or Drama Series. In 1975, Cloris Leachman and Zohra Lampert tied for this award.

Prior to that, there was the Outstanding Single Performance by an Actor and Actress categories, which allowed guest stars to compete alongside performers from TV movies or specials. For example, in 1964, Jack Klugman won for his performance in an episode of The Defenders over Jason Robards in a TV production of the play Abe Lincoln in Illinois. Emmy category confusion seems to go way back!

Kathryn Joosten in Desperate Housewives is my all-time favourite.
